Definition: A mandated benefit is coverage that your health insurance company is required by law to provide. These benefits typically pertain to a specific disease or condition. For example, a common mandated benefit is coverage for diabetes supplies and services. Many states also require health insurance companies to cover mammograms.

Most mandated benefits are established by state legislatures and administrative agencies. Therefore, they vary from state to state.
