Definition:

A personal health record is a history of a patient's medical care, initiated and maintained by the patient herself. This is distinct from electronic medical records (EMRs), which are initiated and maintained by healthcare providers. Personal health records are typically kept in electronic format, but not always. Patients share these records with their healthcare providers in order to improve the quality of care they receive.
