Definition: Your health plan or pharmacy benefits manager (PBM) may require you to obtain prior authorization before they will cover a particular medication. Prior authorization is a formal approval process where your physician must provide a compelling medical reason as to why he or she is prescribing that particular medication, as opposed to a less expensive alternative. This extra step helps plans and PBMs contain costs by steering physicians and patients toward less costly medication, if equally as effective.
